Position: Health Services Case Coordinator Role
Location: South Interlake (MacGregor)

Join Southern Health‑Santé Sud as a Case Coordinator, providing client-focused health services in a multidisciplinary team environment. Use your case management skills to assess and implement client care effectively.

This position reports to the Manager of Health Services and requires a proactive individual with two years of relevant experience. You will collaborate across Home Care services to meet the diverse needs of clients while exercising independent judgment. Your responsibilities include evaluating resources and determining optimal plans to achieve client health outcomes.

Key Responsibilities:

• Conduct assessments to identify client health needs

• Collaborate with multidisciplinary teams for service implementation

• Monitor and evaluate client care options regularly

• Utilize Electronic Home Care Record systems and tools

• Build professional relationships with clients and staff

Requirements:

• Minimum two years of related case management experience

• Accredited post-secondary education in Nursing or Social Sciences

• Active professional license and registration

• Proficient in Microsoft Office and electronic documentation

• Valid Class 5 driver’s license with insurance
