Position: CDE Registered Nurse Role
Location: Northeastern Manitoulin and the Islands

Secure your role as a Registered Nurse with the Diabetes Wellness Team, focusing on diabetes education and chronic disease self-management. This opportunity emphasizes collaborative care and client education.

As a Diabetes Wellness Team member, your expertise as a Certified Diabetes Educator will help you assess and implement effective care plans. You will engage in health promotion and prevention activities, working autonomously under medical directives.

Your role will also involve collaboration with other healthcare professionals to deliver quality service and education.

Key Responsibilities:

• Provide diabetes self-management and education sessions

• Evaluate client care plans and health outcomes

• Collaborate with dietitians to facilitate comprehensive care

• Prepare reports and advance community health strategies

• Consult with community groups on health services

Requirements:

• Bachelor of Science in Nursing

• CDE certification or eligibility to become CDE

• 1-3 years of relevant experience

• Current CPR Level C and First Aid Certification

• Valid driver's license and access to a vehicle
