Lead digital health advancements as a Primary Care Digital Health Analyst. Utilize your analytical and clinical skills to improve care pathways and integrate digital solutions within the community health framework.
This role focuses on the planning, design, and execution of innovative digital tools that align with primary care objectives. The incumbent will collaborate with diverse stakeholders to enhance patient information visibility and streamline workflows across the healthcare continuum. Strong communication skills and the ability to manage multiple priorities are essential.
Key Responsibilities:
• Develop integrated care pathways for chronic disease management
• Co-design digital solutions with healthcare leaders
• Maintain and enhance EMR systems
• Support end-user training and resolve issues
• Participate in collaborative initiatives across services
Requirements:
• Undergraduate degree in health informatics or similar
• 2–5 years of EMR and clinical experience
• Preferred Epic certification within three months
• Project management and analytical skills
• Strong understanding of healthcare workflows
Contribute to quality patient care by driving innovative digital health solutions.
