Enhance resident experiences at people
Care as a Resident and Family Coordinator.
Your role involves managing admissions, providing psycho-social support, and ensuring smooth transitions for current residents. The Resident and Family Coordinator plays a key role in the admissions process while collaborating with interdisciplinary teams. You will prepare applications, track admissions trends, and welcome current residents and families, ensuring their comfort. Engaging with residents during their first weeks is crucial for addressing concerns and fostering belonging.
Support residents’ transitions and enhance their well-being Responsibilities Manage applications for interdisciplinary review
Welcome current residents and families to their rooms
Schedule and facilitate initial care conferences
Provide psycho-social support for social isolation risks
Act as liaison for family and resident complaints
Requirements Registered Social Service Worker (RSSW) designation
Experience in healthcare or long-term care preferred
Excellent communication and organizational skills
Empathetic approach towards residents and families
Understanding of long-term care legislation
