Position: RN - Diabetes Wellness Team (Little Current)
Location: Northeastern Manitoulin and the Islands

Summary  Work within the scope of a Registered Nurse (RN) and Certified Diabetes Educator (CDE) to assess, plan and implement diabetes and chronic disease related self‑management. Work collaboratively with Diabetes Wellness Team (DWT) members as required. Contribute to a shared care model. Work with relative autonomy utilizing medical directives specific to the DWT. Provide self‑management education to clients, families and communities.
Duties   Provide diabetes self‑management education  Providing education for individuals, family, community health promotion and chronic illness prevention

Implement and monitor client care plans  Complete health assessments, plan, implementation, and evaluation of individual care plans to promote self‑management of diabetes and chronic illness
Work alongside the DWT Registered Dietitian to provide a comprehensive health assessment
Review lab values related to chronic disease management
Complete medication reconciliations and provide education with respect to mechanism of action and physiological response
Provide best practice medication recommendations
Collaborate with health care providers and clients to ensure that appropriate management, referral and responses to enquiries are provided

Perform administrative duties in support of the organization’s goals and objectives  Prepare statistical reports for reporting and evaluation purposes
Participate in and promote the interdisciplinary provider model
Assist with the development, implementation, and evaluation of diabetes education and treatment services
Maintain inventory of supplies and place orders to refill inventory as required
Analyze information from individual clients to identify broader implications for health within the communities
Assist with the development and evaluation of policies, protocols, procedures, and advanced directives to improve client care and/or to promote productive utilization of resources
Provide consultation services to community groups, agencies, and health center employees

Perform other duties as assigned  Engage in professional development to maintain current skills and knowledge regarding the scope of the position
Assist in the development and implementation of the organization’s quality improvement initiatives
Network with partner agencies to improve practices, enhance services and move forward shared initiatives relevant to the position

Requirements   Completion of a Bachelor of Science in Nursing degree
Registration in good standing with the College Nurses of Ontario (CNO)
One to three years’ experience working in clinical and/or community
One to three years’ experience delivering diabetes education is an asset
Certified Diabetes Education (CDE) with the Canadian Diabetes Educators Certification Board or be eligible to write and successfully pass the Certification exam and/or working towards completion of the Certificate of Diabetes Education program
Valid Class G driver’s license and access to a vehicle
Clear Vulnerable Sector Check
Current CPR Level C and First Aid Certification Preferred
Membership with Registered Nurses Association of Ontario (RNAO)
Advanced Nursing Foot Care Certification
Venipuncture
Experience working in a community diabetes wellness program
Experience working in Indigenous communities
Experience working within an electronic medical record system
Knowledge   Strong knowledge of diabetes management and risk reduction
Knowledge of the Indigenous communities of Manitoulin Island
Knowledge of the service providers within the Manitoulin Island catchment area
Knowledge of Indigenous health issues
Understanding and sensitivity for the Anishinaabe culture
Skills   Skilled in interactive teaching techniques for groups
Excellent verbal and written communication skills
Skilled in the assessment of clients’ physical and emotional needs is essential
Strong time management skills
Skilled in interpersonal relations
Proficient in Microsoft Office
Ability   Strong ability to provide effective group facilitation
Strong ability to prioritize, plan, organize and schedule
Ability to work with minimal supervision
Ability to speak Anishinaabemowin is an asset
Work Conditions   Potential exposure to biohazards and infectious diseases
Frequent long‑distance travel required
Frequent sitting required
