Elevate resident experiences as a Resident and Family Coordinator s role focuses on admissions management, psycho-social support, and ensuring smooth resident transitions.

In this position, you will coordinate admissions while collaborating with interdisciplinary teams to enhance the well-being of residents. Key tasks involve managing applications, welcoming residents and families, and facilitating care conferences. Your empathetic approach will help address concerns and foster a sense of belonging during residents’ first weeks.

Key Responsibilities:

• Manage applications for interdisciplinary reviews

• Welcome residents and families to their rooms

• Schedule and facilitate initial care conferences

• Provide psycho-social support for tackling social isolation

• Act as liaison for family and resident complaints

Requirements:

• Registered Social Service Worker (RSSW) designation

• Experience in healthcare or long-term care preferred

• Excellent communication and organizational skills

• Empathetic towards residents and families

• Understanding of long-term care legislation
