Taykwa Tagamou Nation is looking for a proactive Health Navigator/NHIB Service Worker to coordinate health services outreach. You will engage with clients compassionately while ensuring they have the resources needed for effective health decision-making. Your insights will help guide enhancements in health navigation within the community.

Key Responsibilities:

• Assist clients in navigating medical appointments and care pathways

• Communicate empathetically and facilitate health literacy

• Co-develop solutions tailored to client needs

• Coordinate e-health services and collaborate with health staff

• Build relationships with community service providers and stakeholders

Requirements:

• Ontario Secondary School Diploma or equivalent education

• Minimum two years’ experience in health or community services

• Strong knowledge of Aboriginal cultural practices

• Valid G driver’s license is required

• Proficiency with productivity software is essential
