Social Service Director - Part Time

Key Responsibilities:

• Establish care plans based on social evaluations

• Provide emotional support to residents and families

• Assist in obtaining community and health resources

Requirements:

• Bachelor’s Degree in Social Work or Human Services

• At least 2 years in a relevant healthcare role

• Preferred MSW for enhanced service delivery

• Knowledge of PCC beneficial

• Solid verbal communication and organizational abilities
