Care Coordinator for Patient-Centered Services

Location: Peterborough County and North Hastings County (Lakefield)

Support patients in Bancroft as a Care Coordinator with Ontario Health atHome. In this role, you'll assess care needs and facilitate access to vital community services for better health.

As a Care Coordinator, you will interact with patients and healthcare teams to identify care needs, create individualized plans, and provide ongoing support. Your expertise will help align patient desires with available resources, ensuring holistic, patient-focused care within a committed organizational culture.

Key Responsibilities:

• Assess patient health needs and service eligibility

• Develop and manage individualized care plans

• Facilitate connections to service providers

• Foster supportive relationships with patients

• Balance available resources with patient preferences

Requirements:

• Membership in an Ontario regulatory body

• Degree in Nursing, Occupational Therapy, or related field

• Minimum 2 years experience in community health

• Proficiency in communication and organizational skills

• Valid driver's license and access to a vehicle
