Position Summary

We are seeking a Full-Time Medical Social Worker to join our non‑profit organization serving Northern Saskatchewan. The successful candidate will provide assessment, counselling, advocacy, crisis intervention, and case management to individuals and families experiencing medical, mental health, emotional, and social challenges. Using a strengths‑based, trauma‑informed, and culturally responsive approach, the Medical Social Worker will connect clients with healthcare, housing, income assistance, education, disability supports, and community resources.

Key Responsibilities
  • Conduct psychosocial assessments.
  • Develop individualized service plans.
  • Coordinate referrals.
  • Collaborate with healthcare professionals and community agencies.
  • Support clients through treatment, recovery, and community transitions.
  • Maintain accurate documentation, case notes, and records.
  • Ensure strict confidentiality in accordance with privacy legislation, ethical guidelines, professional standards, and organizational policies.
  • Exercise sound professional judgment when managing sensitive and confidential information.
Qualifications
  • Bachelor of Social Work from an accredited institution and registration or eligibility for registration with the Saskatchewan Association of Social Workers.
  • Experience in healthcare, social services, mental health, addictions, or community‑based services is preferred.
  • Experience working with Indigenous communities and knowledge of culturally responsive practices are considered assets.
  • Valid Class 5 Driver’s License.
  • Criminal Record Check and Vulnerable Sector Check required.
Salary and Benefits

Annual salary $75,000 - $90,000 based on qualifications and experience.
