Position: Occupational Therapist for Home Care

Make a difference as an Occupational Therapist with CBI Home Health in Peterborough. You will assist clients in achieving independence through personalized, compassionate care while enjoying a flexible work schedule.

In your role, you will be part of CBI Home Health’s Integrated Care Program, helping patients transition safely from hospital to home. Your responsibilities include conducting assessments, providing interventions, and collaborating effectively with care teams to ensure a high standard of health support.

Key Responsibilities:

• Conduct functional assessments to guide OT interventions

• Collaborate with interdisciplinary teams for optimal care

• Implement health coaching and self-management strategies

• Recommend necessary home modifications and adaptive equipment

• Utilize digital platforms for comprehensive care documentation

Requirements:

• Valid registration with the College of Occupational Therapists of Ontario

• Ability to travel within the community with insurance

• Commitment to person- and family-centered support

• Strong critical thinking skills for effective care decisions

• Excellent communication for teamwork and independent efforts
