Key Responsibilities:

• Create personalized rehabilitation and wellness care plans

• Work with a diverse, multidisciplinary team for patient care

• Lead mentorship and engagement within the clinic

• Stay updated with best practices and continuous learning

• Educate patients and families on recovery strategies

Requirements:

• Good standing provincial regulatory college license

• Professional liability insurance as required

• 1–2+ years of outpatient LTC experience preferred

• Willingness to participate in clinic events

• Valid Tuberculin clearance proof required
