Ontario Health Float Care Coordinator

Key Responsibilities:

• Assess and engage patients for service eligibility and planning

• Identify crisis intervention needs through comprehensive assessments

• Develop collaborative care plans with patients and healthcare staff

• Monitor and update care plans for quality assurance

• Manage patient discharges and resource connections

Requirements:

• Registered nursing or relevant healthcare qualification

• Minimum 2 years’ experience in care coordination or similar role

• Familiarity with Champlain region health services

• Completion of COVID-19 vaccination mandatory

• Current N95 Mask Fit Test results required
