We are looking for a dedicated Social Worker for a 0.50 FTE position with a rotating schedule. In this role, you will be responsible for conducting assessments, coordinating treatment, and delivering psychosocial support. Collaborate with healthcare professionals to improve your clients' health outcomes and overall well-being.
Key Responsibilities:
• Conduct comprehensive assessments for clients and families
• Develop and implement targeted care plans
• Provide psychosocial counseling and support services

Requirements:
• Accredited degree in Social Work and active registration
• Experienced in collaborative treatment settings

• Willing to work on a part-time rotating schedule
