Vancouver Coastal Health is recruiting for a Care Coordinator role within the Sunshine Coastal Mental Health team. This position requires a Bachelor's degree and two years of relevant experience, melding clinical expertise with compassionate care. You will coordinate mental health and substance use services, encouraging clients through their recovery journeys with collaborative, strengths-based approaches.

Key Responsibilities:

• Implement crisis intervention strategies effectively

• Develop and execute comprehensive care plans

• Collaborate with community resources and health professionals

• Perform bio-psychosocial and substance use assessments

• Build and sustain trusting client relationships

Requirements:

• Bachelor’s degree in behavioral sciences

• Minimum two years of relevant experience

• Hold a valid BC Driver's License for local travel

• Familiarity with trauma-informed practices and harm reduction

• Proficient in culturally safe communication
