Position: Care Bridge Clinic Pharmacist Role
Location: Central Island (Chemainus)

Become a vital part of the Care Bridge Clinic in Duncan, BC as a Clinical Pharmacist. Focus on patient-centered care within the primary care network and improve medication management practices.

This full-time position at Vancouver Island Health Authority enables you to collaborate with diverse healthcare professionals, ensuring that all aspects of patient safety and quality are met. You will assess and address drug therapy issues, enhance treatment protocols, and educate patients. Travel between clinic sites may be required to meet operational needs.

Key Responsibilities:

• Provide expert clinical pharmacy services to patients

• Collaborate in an inter-professional team for patient safety

• Assess patient needs and develop medication management strategies

• Conduct patient education on medication therapy

• Utilize health technology to manage patient records and data

Requirements:

• Must be licensed by the College of Pharmacists of British Columbia

• Minimum of two years recent experience in a patient care setting

• Valid Class 5 Driver's License is essential

• Strong ability in clinical assessment and teaching

• Excellent communication and teamwork skills
