Join the La Ronge Health Centre as a Full-Time Assessor Coordinator with a focus on Mental Health and Addiction Services. This role involves case management, assessment, and client coordination on a regular shift schedule.

As an Assessor Coordinator, you will play a vital role in the Mental Health Community Clinics & Programs. You will be responsible for screening, assessing, and coordinating initial service requests, while collaborating with interdisciplinary teams for client care. Your expertise will facilitate crisis response and create valuable community resources for effective service delivery.

Key Responsibilities:

• Screen and assess mental health service requests promptly

• Coordinate services and referrals based on client needs

• Facilitate crisis response for clients awaiting service

• Collaborate with program teams on complex case assessments

• Maintain comprehensive records and confidentiality of client data

Requirements:

• Degree in Human Services, Health Sciences, or related field

• Experience working with mental health and addictions clients

• Valid Class 5 driver’s license

• Registered with Saskatchewan Association of Social Workers

• Strong interpersonal and communication skills
