Location: Moosonee

Key Responsibilities:
• Deliver mental health and addiction support to adults
• Facilitate ongoing case management for clients seeking help
• Coordinate care with multidisciplinary teams

• Evaluate progress and adapt services accordingly
Requirements:
• Diploma in Social Service Work or related field
• Experience with First Nations and Indigenous populations
• Knowledge of relevant mental health legislation
• Excellent communication abilities
• Registration with the Ontario College of Social Workers
