Lead health and wellness initiatives as the Health Programs and Services Manager at Tr’ondëk Hwëch’in in Dawson City, Yukon. Oversee community care, chronic disease management, and case planning while supporting Citizens' health needs.

Reporting directly to the Director of Wellness, you'll manage the development and delivery of health programs, enhancing support for individuals facing complex health challenges. This term position focuses on ensuring care coordination, program funding, and community engagement. You will also supervise a dedicated team, promoting programs that align with TH values and strategic goals.

Key Responsibilities:

• Develop and deliver health programs and services

• Coordinate client case management effectively

• Manage complex health case planning and care

• Oversee funding initiatives and budget administration

• Engage with governments and community partners

Requirements:

• Post-secondary degree in health-related field

• Five years of management experience in health services

• Knowledge of Indigenous health programs and resources

• Experience collaborating with First Nations governments

• Trauma-informed service delivery expertise
