In this full-time Care Coordinator position, you will assess individual care needs and develop necessary service plans in collaboration with patients and their families.

Your role includes coordinating multimedia care approaches while ensuring efficient communication among healthcare practitioners. Experience and a foundational understanding of community resources are integral to meeting patient needs effectively.

Key Responsibilities:

• Collaborate to assess care needs and service eligibility

• Develop tailored care plans for individual patients

• Link patients with appropriate service providers

• Monitor delivery of care plans with efficiency

• Facilitate respectful relationships with patients and families

Requirements:

• Membership in a relevant regulatory body

• Degree in Nursing, Therapy, or Social Work

• Minimum 2 years in community health

• Excellent organizational skills
