In this vital term role, you'll oversee the implementation of home and community care services, ensuring effective case management for complex health issues. Reporting to the Director of Wellness, you will coordinate health-related programs while managing a team and liaising with community partners for strategic health initiatives.

Your role will significantly shape the quality of care and support available.

Key Responsibilities:

• Oversee the delivery of health programs and services

• Support management of client case coordination

• Lead complex health cases and case planning

• Administer budgets and funding initiatives

• Collaborate with community and government agencies

Requirements:

• Degree in nursing or health administration

• Minimum five years of health program management

• Understanding of community health needs and resources

• Experience in First Nations health collaborations

• Knowledge of culturally safe service delivery
