As a licensed Social Worker, you will utilize two years of relevant experience to assist clients in navigating complex health-related issues.

Your role will involve assessments, crisis intervention, and education to enhance client autonomy and decision-making. You'll work closely with multidisciplinary teams to ensure comprehensive care.

Key Responsibilities:

• Facilitate client identification and resource coordination

• Provide assessments and support for health issues

• Implement crisis intervention strategies as needed

• Educate clients and families about health promotion

• Maintain documentation as per health care standards

Requirements:

• Eligible for licensure as a Social Worker in Manitoba

• Minimum two years of experience in health care

• Proficient with Microsoft Office and technological tools

• Strong problem-solving and communication abilities

• Valid Class 5 Driver’s License and access to personal vehicle
