Location: Rideau Lakes area (Kemptville)
Transform patient care journeys with Ontario Health atHome as a Float Hospital Care Coordinator. This role focuses on the assessment, planning, and provision of services to meet patients' needs.
As a Float Hospital Care Coordinator, you will play a key role in developing comprehensive care plans tailored to individual patients, facilitating a smooth transition from hospital to home. Our collaborative team approach reinforces the importance of connecting with patients and their families while utilizing available community resources for quality healthcare delivery. Dynamic engagement ensures that patient needs are met effectively in various settings, including home care and hospital environments.
Key Responsibilities:
• Engage and assess clients for healthcare program admissions
• Establish care plans based on client assessments
• Implement and monitor client care initiatives
• Reassess services regularly to ensure quality care
• Manage discharge planning and resource redirecting
Requirements:
• Degree in Nursing, Physiotherapy, or Social Work required
• At least two years of related healthcare experience
• Knowledge of local health care delivery systems
• Expected COVID-19 vaccination as per policy
• Bilingualism (English and French) is beneficial
