Nutrition and Diabetes Care Coordinator

Location: Northwestern Ontario (Red Lake)

Enhance client health through effective diabetes management as a Nutrition and Diabetes Care Coordinator. Collaborate with an interdisciplinary team to deliver exceptional care and nutritional support to clients.

As a key member of the care team, you will assess, educate, and support clients living with diabetes. This includes developing and implementing teaching initiatives tailored to individual needs, as well as acting as a resource for clients and healthcare providers. Strong communication and community engagement will be vital in this role.

Key Responsibilities:

• Develop interdisciplinary diabetes education programs

• Participate in nutrition workshops for client engagement

• Counsel clients on personal nutritional assessments

• Foster effective community and partner relations

• Maintain comprehensive records and documentation

Requirements:

• Bachelor’s Degree in Applied Nutrition or equivalent

• Registration eligibility with College of Dietitians of Ontario

• Certification as a Diabetes Educator required within one year

• Preferred membership in Diabetes Educators Section

• Commitment to ongoing professional development
