Rainbow Valley Community Health Centre (RVCHC) is in the Ottawa Valley in the village of Killaloe, Ontario. RVCHC is the only Community Health Centre in Ontario to be administered by a hospital, St. Francis Memorial Hospital (SFMH). The team is comprised of part time staff including Physicians, Nurse Practitioners, Registered Nurses, administrative staff and Social Worker.

Pay Grid: $47.74 - $51.70 /hour

The Rainbow Valley Community Health Centre Social Worker functions as a member of the primary health team in the provision of assessment, individual and family counseling, case management and facilitation of groups operating within the values and objectives of the Centre. Crisis intervention services, in partnership with the other mental health programs in the County, are also an element of the position.

The Social Worker works closely within the inter-disciplinary team, providing consultation to members of the team.

RESPONSIBILITIES:

• Supports the functioning and independence of individuals and families by providing counseling services to individuals and families.
• Provides information and advice to clients of the Centre and residents of the catchment area who are seeking support for mental health issues, and/or issues impacting on their ability to cope.
• Participates in case conferences, and provide case management to clients and families as required.
• Provides mental health consultations and information to staff of the Centre.
• Develops and implements mental health support groups, mental health promotion groups and groups that support clients with mental health issues.
• Ensures appropriate interventions are delivered by maintaining proper client records, assessment notes and correspondence.
• Ensures appropriate encountering of services in the Centre’s electronic charting system.
• Contributes to the development of new programs that address community health needs by collaborating with Centre staff and other agencies to identify, recommend and/or develop resources and materials and by participating in the delivery of programs as required.
• Provides leadership for appropriate community educational programs in collaboration with Centre staff and partner agencies.
• Attends and participates in regular staff and team meetings, and staff & board events.
• Participates in the selection and supervision of students and professionals seeking placements pertaining to counseling at the Centre.
• Identifies appropriate clients who could benefit from participation in the LHIN’s Health Links approach. Works with clients to develop Coordinated Care Plan to meet RVCHC annual target.
• Participates in the development of relevant policies, procedures and best practices for the Centre.
• Initiates and maintains partnerships with relevant community agencies and professionals.
• Promotes community awareness of and participation in the Centre’s programs.
• Provides support and supervision to students who have placements at the CHC.
• Plans and participates in outreach activities related to primary health care.
• Works in a manner that preserves confidentiality, seeks to minimize risk, incorporates health promotion and recognizes the determinants of health.
