Case Manager for Mental Health Services

As a Case Manager, your main responsibility will be to support individuals with mental health challenges, ensuring access to the right services. You will work closely with community organizations and act as a liaison to develop support systems. Key Responsibilities:

• Facilitate client releases of information for care compliance

• Provide crisis intervention and assist in care planning

• Conduct regular home visits to assess client situations

• Keep detailed and current client records as per standards

• Prepare public presentations on mental health programs

Requirements:

• Bachelor’s degree in a relevant field such as Social Work

• Successful completion of a clean drug screen

• DMV background check pass required

• Ability to engage culturally diverse patients
