In this pivotal role, you will assess patient care needs and develop tailored service plans. Engage with patients, families, and healthcare professionals while ensuring seamless access to health services. Your contributions will enhance the wellness of individuals within the community, supported by our extensive network of over 9,000 professionals.

Key Responsibilities:

• Assess care needs with patients and families

• Develop and monitor individualized care plans

• Link patients to appropriate service providers

• Establish helping relationships with patients

• Balance patient needs with available resources

Requirements:

• Membership in a relevant regulatory body

• Recent community health experience

• Excellent communication and organizational skills

• Valid driver’s licence and reliable vehicle

• Proficient in Windows; bilingual in English and French
