Position: Social Worker II for Assisted Living
Location: Smoky Lake

Elevate patient care at Assisted Living Alberta as a Social Worker II in Smoky Lake. Provide vital psychosocial evaluations, counseling, and case management across care settings.

This part-time role requires strong clinical judgment and adaptability in a collaborative healthcare environment. You will support patient transitions between Acute Care, Home and Community Care, and Continuing Care Homes. Foster meaningful relationships with patients and families through effective resource navigation and counseling techniques.

Key Responsibilities:

• Conduct psychosocial assessments for patients and families

• Provide individual, group, and family counseling services

• Manage case coordination and discharge planning

• Screen for risk and provide proactive interventions

• Promote public awareness of available health resources

Requirements:

• Bachelor’s degree in Social Work required

• Active registration or eligibility with Alberta College of Social Workers

• Basic Life Support certification for healthcare providers

• Driver’s license and reliable vehicle for travel

• Minimum one year experience in related care settings
