Medical Social Worker

Position: Medical Social Worker at St Luke's ElderCare

Provide essential psychosocial support as a Medical Social Worker with St Luke's Elder Care. In this role, you'll assist elders and their caregivers with assessments and case management to ensure dignity in care.

As a vital member of our team, you will conduct thorough BPSS assessments and coordinate care for referred elders. Collaborating with a multi-disciplinary team, you will develop discharge planning and provide community case management services.

Your role is crucial for empowering elders while ensuring proper documentation of case files and reports.

Key Responsibilities:

• Manage social work caseload effectively and promptly

• Conduct BPSS assessments for elders and caregivers

• Collaborate in multidisciplinary meetings for better care

• Coordinate discharge planning for elders transitioning services

• Support therapeutic groupwork and ACP conversations

Requirements:

• Degree or Graduate Diploma in Social Work

• Minimum 2 years direct social work experience

• Experience with elders in healthcare or community settings

• Proficiency in Microsoft Office applications

• Ability to converse in local languages
