Primary Care Nurse Practitioner in Ontario

Location: Renfrew County and Lanark Highlands Township (Deep River)

Join the Peterborough Family Health Team as a Primary Care Nurse Practitioner and make a meaningful impact in community health. This 1-year contract role involves working 30 hours per week, enriching patient care.

As a Nurse Practitioner, you will operate within your full scope of practice, delivering essential health services. Your responsibilities will include performing health screenings, managing chronic illnesses, and consulting with physicians to ensure comprehensive patient care.

Key Responsibilities:

• Provide wellness care and health screenings for patients

• Monitor chronic illnesses and implement treatment plans

• Diagnose health issues and determine appropriate interventions

• Gather and document health-related statistics as needed

• Participate in Family Health Team meetings regularly

Requirements:

• RN registration in the Extended Class with the College of Nurses of Ontario

• Completion of accredited Nurse Practitioner program

• 3 to 5 years of experience in primary health care settings

• Willingness to work evenings or weekends as needed

• Dedication to supporting vaccination efforts
