Mobile Clinic Nurse Practitioner

Location: Deseronto

Become a Nurse Practitioner with Tsi Kanonhkhwatsheri:yo IIPCT in Deseronto, Ontario. This full-time role focuses on delivering comprehensive healthcare in a mobile clinic, catering to Indigenous populations with compassionate, culturally sensitive care.

In this position, you will be essential in managing chronic conditions and providing preventive health education within the community. As a Nurse Practitioner, you will coordinate care with a dedicated team of healthcare professionals, ensuring patients receive holistic treatment. Your expertise will help improve health outcomes and enhance well-being across diverse populations.

Key Responsibilities:

• Provide acute and chronic illness management

• Develop and implement personalized health care plans

• Conduct health assessments, including physical exams

• Collaborate with team members for optimal patient support

• Participate in community outreach and education programs

Requirements:

• Current registration as RN(EC) with the College of Nurses of Ontario

• Minimum two years of Nurse Practitioner experience

• Understanding of Indigenous culture and healthcare

• Proficient in MS Office and EMR systems

• Must pass a Vulnerable Sector check
