Porter is expanding its services into Michigan and seeks dedicated Nurse Practitioners for its growing team. In this role, you will conduct comprehensive health risk assessments for members, evaluating medical histories and vital signs. Your expertise will guide care management through personalized, evidence-based practices while ensuring transparency and compassion throughout the healthcare continuum.

Key Responsibilities:

• Conduct in-home health risk assessments

• Evaluate past medical history and vital signs

• Perform point-of-care testing as needed

• Formulate lists of medical conditions for care management

• Collaborate with Porter Care Guides for patient care solutions

Requirements:

• Valid unencumbered Michigan NP license

• 3 years of patient care experience required

• Experience in primary care and EMR skills preferred

• Strong empathy and exceptional communication skills

• Ability to engage with patients compassionately
