Role Profile
- Registered Practical Nurse (RPN)

Mission

Reporting to the Director of Care, the Registered Practical Nurse (RPN) is responsible for providing resident‑centered and goal‑oriented nursing care in accordance with standards of practice and provincial legislation. Working collaboratively within the interdisciplinary team, the RPN will participate in the development, implementation, monitoring and evaluation of care and clinical services to enhance the provision of resident care.

Key Responsibilities
  • Maintains accurate and current health records, clinical assessments and documentation, reflecting the resident’s abilities, needs and preferences and in accordance with the College of Nurses standards and organizational policies and procedures
  • Implements and evaluates the effectiveness of care and services as outlined in the plan of care, while making appropriate revisions to goals and interventions in collaboration with the interdisciplinary team as required
  • Assists with scheduling and participating in resident care conferences to ensure coordination of services where applicable
  • Works with the Director of Care or designate to facilitate resident move‑in, discharge and transfers to and from the home
  • Performs necessary nursing functions to facilitate the delivery of safe resident care, including clinical assessment, medication administration, care treatments and maintaining documentation of care provided in compliance with regulatory requirements and organizational policies and procedures
  • Processes physician orders, ensures drugs and narcotics are secured and administered as ordered or discarded as required in compliance with all regulatory requirements, clinical scope and organizational policies and procedures
  • Works with preferred pharmacy regarding resident orders and to ensure appropriate inventory of emergency drugs where applicable
  • Facilitates and provides resident/family education related to resident conditions, care and services where required
  • Plans, coordinates, manages and evaluates the resident’s plan of care routinely and as required, working with the resident/substitute decision maker, physicians, interdisciplinary team and contracted service providers where applicable
  • Coordinates resident care assignments and ensures safe, efficient and timely delivery of care services
  • Responds to and manages significant changes in residents’ condition and/or medical emergencies; provides emergency first aid
  • Maintains resident safety and works in a manner that minimizes risk of adverse outcomes
  • Supervises, trains, delegates and directs unregulated care providers as required
  • Responds to resident/family concerns immediately and communicates to the Director of Care/Executive Director and other departments as applicable to ensure appropriate follow‑up and corrective action
  • Assists the Director of Care in the coordination, implementation and monitoring of Infection Prevention and Control, Health and Safety and clinical programs and quality/safety plans for the nursing department
  • Works collaboratively with the interdisciplinary team in the development, implementation and sustainability of clinical and quality improvement measures and programs
  • Works in a manner that maintains resident rights, dignity and preserves confidentiality
  • Upholds and promotes core values and philosophy relating to ethics, morality, and integrity as set out in the code of conduct
  • Completes all other duties as assigned
  • Adheres to responsibilities and requirements for reporting adverse resident occurrences subject to regulatory and legislative direction
Required Education, Credentials And Experience
  • Current registration as a Registered Practical Nurse with the applicable provincial College of Nurses
