Candidates should be Vent and Trach trained, demonstrating the ability to thrive in dynamic, fast-paced environments. Key Responsibilities:

• Coordinate and evaluate patient care within the team

• Deliver care to clients in their homes and facilities

• Observe, document, and address health condition changes

• Foster positive relationships with clients and healthcare teams

• Actively engage in community health efforts and training

Requirements:

• Graduation from a Nursing program (RN or RPN)

• At least one year of related nursing experience

• Certification with the College of Nurses of Ontario

• Valid First Aid and CPR Level HCP certification

• Possess a valid driver’s license and reliable vehicle
