LAMP Community Health Centre is looking for a passionate Registered Practical Nurse committed to primary health care. With a focus on delivering clinical assessments and health teaching, you will help manage patient care in a busy office. Leverage your problem-solving skills and communication expertise to ensure effective service navigation for clients in the community.

Key Responsibilities:

• Conduct assessments and assist with health exams

• Manage intake and triage for routine patient needs

• Provide monitoring and education for patients with complex needs

• Promote health initiatives and improve patient self-care

• Document care accurately and assist with referrals and procedures

Requirements:

• Registered and in good standing with the College of Nurses of Ontario

• At least two years of community nursing experience

• Strong initiative and ability to work independently

• Excellent interpersonal and communication skills

• Valid driver’s license preferred
