As a Nurse Practitioner, your role includes working under the Primary Care Manager to improve client health outcomes. You are expected to engage in patient-centered care and have solid communication and decision-making skills. Your knowledge in chronic disease management and preventative care will be vitally important in this role.

Key Responsibilities:

• Collaborate closely with Primary Care Team members

• Promote patient values and community health

• Tackle chronic disease management effectively

• Issue and manage prescriptions within scope

• Engage with traditional and medical healing practices

Requirements:

• Master’s degree in nursing with NP extended class

• Current Nurse Practitioner registration

• Minimum of two years of primary care experience

• Valid Ontario Driver's License for travel

• Certification in CPR and First Aid required
