About the position

Duties

• Performs comprehensive health assessments, diagnoses health/illness conditions, and treats, monitors, manages and evaluates acute and chronic illnesses.

• Orders and interprets screening and diagnostic tests, performs medical procedures, prescribes medications and makes referrals to other specialist medical providers or programs.

• Supports, educates, coaches and counsels clients regarding diagnosis, prognosis and condition self management.

• Develops, utilizes and evaluates processes within the practice setting to ensure that clients receive coordinated health services; collaborates with members of the multi­disciplinary health services team to provide and promote inter-professional client­ centered evidence-based care at the individual, organization and system level and advocates on the client's behalf.

• Assesses, identifies and critically analyzes information from a variety of sources to determine trends and patterns that have health implications; guides, initiates and provides leadership in the development and implementation of standards, practice guidelines, quality assurance and education and research initiatives.

• Initiates or participates in the design of services/interventions for health promotion, health protection and the prevention of injury, illness, disease and complications.

• Work collaboratively with other health service providers such as physicians, nurses, psychologists, psychiatrists, dieticians, administrative assistants as well as staff outside of health services such as Correctional Officers for effective delivery of services.
Work environment
Correctional Service Canada (CSC) is the federal government agency responsible for administering court imposed sentences of a term of two years or more. CSC manages various security level institutions and supervises offenders under conditional release in the community. CSC's mandate is to contribute to public safety by actively encouraging and assisting offenders to become law-abiding citizens, while exercising reasonable, safe, secure and human control.
