Access Healthcare Services is seeking Registered Practical Nurses or Registered Nurses passionate about community care. Successful candidates will provide high-quality client-centered care and perform comprehensive assessments to develop holistic care plans in accordance with CNO standards. This position also involves collaboration with interdisciplinary healthcare teams and participation in the on-call nursing rotation.

Key Responsibilities:

• Provide high-quality in-home client-centered nursing care

• Conduct comprehensive client assessments and plan care collaboratively

• Adhere to CNO standards and AHS policies

• Uphold infection prevention and control measures

• Engage in ongoing education and professional development

Requirements:

• RN or RPN in good standing with CNO

• Proof of Covid-19 vaccination

• Valid driver's license and vehicle required

• Current CPR certification

• Recent Step 2 TB Test and police check
