What You’ll Do:

• Conduct wellness assessments and monitor resident health changes
• Support medication management and perform MAR/TAR audits
• Communicate with families, physicians and care teams
• Monitor incidents, falls and follow-up care
• Maintain documentation and compliance with regulations
• Support infection control and clinical quality initiatives
• Train and support care team members

What You’ll Bring :
• Active LPN/LVN license
• 1+ year experience in senior living or clinical setting
• Strong assessment and documentation skills
• Knowledge of medication administration and compliance
