Position: Fraser Health Community Care Nurse
Location: Harrison Lake Region (Agassiz)

Key Responsibilities:

• Establish collaborative therapeutic relationships with clients

• Develop and document individualized client assessments

• Provide direct nursing care and health education initiatives

• Monitor care plan effectiveness and adjust as necessary

• Accurately document client interactions and care provided

Requirements:

• Registered Nurse eligible with BCCNM in BC

• At least 2 years of experience in geriatric nursing

• Valid provincial driver's license for work-related travel

• Ability to communicate effectively and mentor others

• Strong organizational skills for case management
