As a Primary Care Nurse (PCN), you will function under the guidance of the Manager while following established nursing standards from the BC College of Nurses and Midwives. In this position, you will focus on providing direct clinical support, collaborating with an interprofessional team to devise personalized care plans that reflect patients' goals and needs.

Key Responsibilities:

• Provide direct clinical care to diverse patient populations

• Work collaboratively with healthcare teams on care plans

• Ensure safe and ethical practices in patient interactions

• Adapt care plans based on individual patient goals

• Uphold BCCNM ethical standards and nursing protocols

Requirements:

• Active RN registration with BCCNM

• Strong interprofessional collaboration skills
