Nurse Practitioner in Palliative Care

Join Ontario Health atHome as a Nurse Practitioner focused on palliative care in the Erie St. Clair Region. Provide essential patient-centered services across various settings, from homes to hospitals.

This full-time role requires an experienced Nurse Practitioner to assess patient care needs, offer community service referrals, and engage effectively with patients and caregivers. You will deliver palliative consultation services and work collaboratively within a dedicated team to improve patient trajectory. Attention to detail and excellent time management are vital for this position.

Key Responsibilities:

• Deliver Nurse Practitioner services for palliative care patients

• Diagnose and manage chronic disease symptoms

• Partner with Hospice for comprehensive assessments

• Travel between various care settings in the region

• Ensure adherence to professional and health regulations

Requirements:

• Registered Nurse or Nurse Practitioner with current registration

• Knowledge of community health resources

• Specialized education in palliative care management

• Valid Ontario Driver’s License and access to a vehicle

• Required COVID-19 vaccination prior to start date
