The Peterborough Family Health Team is seeking RPNs for contract positions ranging from 30 to 37.5 hours per week, emphasizing preventive health initiatives and chronic disease monitoring. This role is ideal for health professionals committed to high-quality, person-centered care while maintaining patient confidentiality through secure documentation practices.

Key Responsibilities:

• Conduct population health reviews via EMR

• Coordinate preventive care initiatives, including immunizations

• Reconcile referrals and consultations accurately

• Contact patients for appointment scheduling and follow-up

• Support physicians with chart reviews and clinical information

Requirements:

• Diploma in Practical Nursing with RPN registration

• Minimum 3 years clinical experience, preferably in primary care

• Proficient in Primary Care EMR systems

• Strong communication and interpersonal skills
