This position requires delivering high-quality individualized care to enhance residents' comfort and well-being. You'll work closely with families to manage pain, symptoms, and emotional support during critical times. This full-time role includes 12-hour rotating shifts from October 2026 to November 2027.

Key Responsibilities:

• Implement individualized hospice care for residents

• Assess pain and symptom management strategies

• Collaborate with families for psychosocial support

• Participate in interdisciplinary care planning

• Maintain accurate care records and documentation

Requirements:

• Registered with the College of Nurses of Ontario

• BCLS certification and palliative care course preferred

• 1-2 years of relevant palliative care experience

• Two Step TB test and a Vulnerable Sector Check

• Commitment to compassionate end-of-life care and ongoing growth
