Primary Care Nurse Practitioner - WCHC

Location: Wellesley

Join Woolwich Community Health Centre as a Primary Care Nurse Practitioner, working a part-time, 22.50 hours weekly, at the Wellesley site. This role prioritizes delivering primary health care and supporting community health initiatives.

As a temporary nurse practitioner for six months, you will play a key role in reducing primary care attachment rates. Candidates should possess a BScN and Primary Health Care Nurse Practitioner certification, along with 2-5 years of relevant experience. Your contributions in wellness care, assessment, and collaboration within a team setting will be vital for client health outcomes.

Key Responsibilities:

• Deliver primary health care services to registered clients

• Participate in community health programming initiatives

• Work closely within an interdisciplinary health team

• Manage chronic diseases and episodic conditions

• Perform home visits when necessary

Requirements:

• BScN and certification as a Primary Health Care Nurse Practitioner

• 2-5 years' experience in a primary care context

• Proficient communication and organizational skills

• Willingness to work evenings and partake in on-call duties

• Experience with electronic medical records preferred
