WCHC Nurse Practitioner - Community Health Focus

Location: Wellesley

Enhance community health as a Nurse Practitioner at the Woolwich Community Health Centre, providing care at the Wellesley site for 22.50 hours per week. You will engage in comprehensive health care and community programming to improve access.

This is a temporary part-time position lasting six months, emphasizing teamwork in an interdisciplinary setting. Qualified candidates will have a BScN with a Nurse Practitioner certification and relevant experience in primary health care (2-5 years). Your expertise in chronic disease management and effective communication will be crucial in serving the diverse community.

Key Responsibilities:

• Provide comprehensive healthcare to clients in an interdisciplinary team

• Engage in community-focused health initiatives

• Manage chronic diseases and episodic health issues

• Conduct necessary home visits for clients

• Collaborate with various health professionals

Requirements:

• BScN and Primary Health Care Nurse Practitioner qualifications

• 2-5 years’ nursing experience in primary care settings

• Strong problem-solving and interpersonal skills

• Flexibility for evening shifts and on-call duties

• Computer literacy with prior EMR experience preferred
