As a Wellness Nurse (LPN/LVN), you support resident health, monitor care needs and ensure safe medication practices and clinical compliance.
What You'll Do   Conduct wellness assessments and monitor resident health changes
Support medication management and perform MAR/TAR audits
Communicate with families, physicians and care teams
Monitor incidents, falls and follow-up care
Maintain documentation and compliance with regulations
Support infection control and clinical quality initiatives
Train and support care team members
What You'll Bring   Active LPN/LVN license
1+ year experience in senior living or clinical setting
Strong assessment and documentation skills
Knowledge of medication administration and compliance
Strong communication and problem-solving skills
