The position is based at Eriksdale Wellness Centre, demanding a Bachelor of Nursing and active Manitoba RN registration. Key Responsibilities:

• Collaborate with healthcare teams for client-centered care

• Create and implement chronic disease education programs

• Assess and intervene in diabetes and cardiovascular health

• Maintain quality client care through EMR systems

Requirements:

• Bachelor of Nursing and RN registration in Manitoba

• Chronic Disease Educator certification preferred

• Minimum two years chronic disease nursing experience

• Advanced communication skills in diverse settings

• Current Basic Life Support certification required
