Come join us! As a result of internal growth at SFMH, we are hiring a casual Registered Nurses with our Cardiac Rehab and Diabetic Clinic.

Pay Grid: $41.15 - $58.98 /hour

Through organizational readiness, the Cardiac Rehab/Diabetic Educator/Admin RN plans, develops and promotes Cardiac Rehab and Secondary Prevention services and the Regional Diabetes Program services for the communities served by SFMH.

Duties will include but are not limited to:

Develop and implement clinic specific policies and procedures to guide practice and processes, and collaborate with partners in the development.

Make recommendations for equipment requirements to the Director Patient Care Services.

Demonstrate ability to communicate effectively and with respect in an oral and written format with patient, colleagues and other health disciplines to ensure effective, efficient and appropriate use of services.

Schedule and promote clinical and educational events in conjunction in UOHI and PRH using OTN ncis Memorial Hospital.

Process new referrals received by Family Physicians, Emergency doctors or clinicians, or the UOHI – including initial assessment and determine patient qualification for programs available.

On-going patient assessments during participation in either program and attend to clinical changes as appropriate.

Ensure discharge planning and follow-up sessions are scheduled and completed.

Coordinate with Community partners (UOHI Cardiology, Family Physicians, Telehealth, Diabetic Clinic, Smoking Cessation, Heart-Wise Community groups, CCAC and Home Support) to promote self-management.

Develop evaluation tools – Clinical performance, patient satisfaction etc.

Organizes and provides diabetes education for patients/families, the public and health care professionals.

Develop and manage data collection for reporting, patient management, program evaluation and risk stratification.

Completes, audits and submits RAI-MDS assessments quarterly; responsible for timely completion of assessments on admission, with change of condition and discharge.

Qualifications:

Current registration and a member in good standing with the College of Nurses of Ontario

Must be cognizant of and function in accordance with relevant legislation (RHPA) and Professional Standards of Nursing Practice for Registered Nurse (RN)

Current BCLS, ACLS, DCA, 12 Lead ECG interpretation and Coronary Care

Demonstrated ability to represent SFMH at Regional Level (Satellite to Partner sites)

Experience with chronic disease management and patient education using various modalities required

Current Telemedicine / Diabetic Care / Cardiac care experience is preferred

Experience developing and implementing policies and procedures considered an asset

Member in good standing of the Diabetes Association of Canada
