Key Responsibilities:

• Develop personalized Care Plans for patients

• Deliver comprehensive nursing services during home visits

Requirements:

• Degree or diploma in nursing (RN or RPN)

• Current registration with the College of Nurses of Ontario

• Passionate about patient advocacy and support

• Valid driver’s license for travel

• CPR certification and background checks required
